ALL OF THE BELOW FORMS MUST BE COMPLETED AND ON FILE AT THE LOCATION OF YOUR CHOICE BEFORE YOUR CHILD IS CONSIDERED REGISTERED AND ABLE TO ATTEND ONE OF OUR FACILITIES. FIELD TRIPS CONSENT /RELEASE

Our program includes outdoor play, parks, fieldtrips, and spontaneous outings. Fieldtrips are scheduled for our preschoolers or during school closings and holiday vacations. Parents will be informed of any scheduled fieldtrips in advance and will be asked to give their written consent. We often ask parents to participate in these activities to ensure the safety of the children by increasing the adult to child ratio. Upon enrollment, you give consent for your children to be included in off-site activities and spontaneous outings. You understand that Kreayola Kids Centers will notify all parties by letter, verbally, or posted sign of any off-site trip at least 24 hours in advance. In addition, the parent understands that Kreayola Kids Centers and staff will take all necessary precautions to ensure the safety of your child. So in consideration of the opportunities for your child to participate, and fully recognized that such an undertaking involves an element of risk, the parent assumes all risks and hazards incidental to such participation, an.d do hereby release, absolve, indemnify, and agree to hold harmless Kreayola Kids Centers, Kreayola Kids, LLC, Valencia Howard-Makel, Robert Makel of Suitland, MD, Kreayola Kids staff and chaperones, leaders, sponsors, and persons transporting your child to and from these activities. Neither Kreayola Kids, LLC/Valencia Howard-Makel and Robert Makel of Suitland, MD, nor any of said parties above shall be held financially responsible for any injury, illness of any kind, or death incurred as a direct and indirect result of any activity. If you do not wish for your child to participate in a scheduled event, based upon availability, your child will be allowed to remain in the facility under staff care. However, parents must have an alternate plan.

PHOTOGRAPHY WAIVER & RELEASE

You hereby grant permission to Kreayola Kids, LLC to use any photograph or digital image of your children or the minor children for whom you are the legal guardian, in connection with slideshows, newsletter articles, advertisements, and publicity of any of the activities and programs conducted at our Centers. There shall be no obligation on the part of Kreayola Kids to notify you in advance whenever any such photograph or digital image is to be used. You waive any right to inspect or approve any photograph or digital image of your children that may be used by Kreayola Kids pursuant to this Release, now or in the future. You further waive any right to compensation from Kreayola Kids on behalf of yourself and your children in connection with the use of any photograph or digital image of ·your children pursuant to this Release. You hereby agree to release and hold Kreayola Kids harmless from and against any claims, damages, or liability arising from or related to the use of such photographs or digital images pursuant to this Release.
